Support When You Need It Most

Hopedale Medical Foundation is a not-for-profit charity corporation. If you or your family member is facing financial uncertainty, our staff can talk with you more about our financial assistance.  Patients and residents can receive discounts or free services through the program.

  • All must apply and qualify based solely on the individual or family’s gross income and available assets.
  • People must not qualify for other assistance programs such as public aid.
  • Must not be receiving the same or similar services elsewhere.

For those that apply and qualify for financial assistance, you will be notified as soon as possible regarding the amount of the discount and setting up a payment plan. HMC reserves the right to reconsider approval if we receive information that a family’s financial situation has changed, or false information was provided or omitted.

How To Apply

To receive financial assistance, you (or your guardian/Power of Attorney) must fill out an application and submit it with the requested paperwork and financial documentation to:

Special Note

Financial assistance does not apply to private physicians’ services.  These physicians are independent contractors. If you need assistance with paying your physician’s bill, please contact their offices directly.

No person will be denied access to emergency services based on their inability to pay.
